Affidavit of Support and Consent Sample for a Travelling Minor

Hopefully, this Affidavit of Support and Consent sample can help parents get passports for their children. We looked for sample affidavits online, and couldn’t find any. We ended up making our own affidavit and having it notarized in Naga (for PHP50) before heading over to Legazpi. Looks like we had some missing information in ours – so we still had to make use of the services of the Affidavit Desk at the DFA (for PHP300). Here is the text of the correct affidavit that was put together by the DFA’s Affidavit Desk. It could help some of you save a couple of hundred pesos (and time too!)

AFFIDAVIT OF SUPPORT AND CONSENT

We spouses, [NAMES OF PARENTS]. both of legal ages, married, Filipinos, and residents of [ADDRESS], under oath, do hereby depose and state:

1. That we are the biological and legitimate parents of [NAME OF TRAVELLING MINOR], minor passport applicants with the Department of Foreign Affairs in Legazpi City;

2. That our child will be travelling abroad with us anytime;

3. That we likewise guarantee for the safety of our child and we are giving our full support and consent for the said travel;

4. That we are executing this affidavit to attest the veracity of the foregoing facts and for whatever legal purposes this may serve.

IN WITNESS WHEREOF, we have set our hands this [DATE] at [PLACE]

[NAMES OF PARENTS OVER SIGNATURE]

SUBSCRIBED AND SWORN before me in the City of [PLACE] this [DATE] by [NAMES OF PARENTS] who satisfactorily prove to me through his/her Philippine Passport No. [PASSPORT NUMBER] issued by the Department of Foreign Affairs in [PLACE OF ISSUE], personally known to be the same person who executed the foregoing affidavit and acknowledged that he/she executed the same.

    • Hi! When we applied for our baby’s passport, we brought identification for both my husband and myself. From what I understand, it doesn’t have to be a passport for both – but you’ll need the passport of the adult that will accompany the child when traveling.       For the affidavit, I think any form of government issued identification should work (drivers license, sss, tin, etc) – but what we used were our passports.

      • I read thsi in a forum:

        “The “a Notarized Affidavit of Support and Consent to travel from either parent (if minor is a legitimate child) / from mother (if minor is an illegitimate child)”
        is only applicable if she will not be travelling with the kid..

        How true is this?

        • Hi Raymund. I don’t know how true that is. We brought an affidavit with us when we applied. It needed corrections too (which is why we posted the sample). But we (my husband and I) were both to accompany our baby during travel.

    • Hi Yhen. From how we understood it, it is needed even if the child travels with parents. It would be good to have one just in case.

  4. Hi. FYI, I applied for my son’s passport last Saturday at DFA NCR East (Megamall), I just submitted the original copy of Birth Certificate from the NSO, and photocopy of my passport then showed my original passport that’s all no need for affidavit of support consent. Hope this one helps.

    • Hi! Thanks for letting us know. I wish the DFA offices were consistent with their requirements. It’ll save parents time and money if the affidavit really isn’t necessary.

  5. Hi, just want to clarify, what if parents are also applying passport together with their child for the first time? Base on the DFA list of requirements, parents should present their passport? How is it?

    • Hi Almira. I don’t think it should be a problem since you’ll be bringing the necessary documents to get your passport. 🙂

  6. Hello there! Just want to ask if it would cause a problem if the date of marriage on the couple’s marriage certificate is different from what is on the child’s birth certificate? Will I still be able to get my child’s passport?

    • Hi, Kath! I am not so sure. Hmm. If they allow children with parents who are not married to apply for a passport, then they should allow your child to get a passport too. Just make sure you (As the mother) have all the necessary documents too. Hopefully, it won’t be a problem though. My guess is, they’ll just ask for another affidavit.

    • hi cecile. The affidavit is a document that you (or you can have it done at DFA, since they have those conveniently located staff that charge for their services) will supply, and have notarized. The sample that we have online is something you can copy and fill in with the child’s (and the parent’s) information. If the child is travelling without the parents, I’m not sure what other documents will need or where you’ll get them. Good luck!

  8. Hello.. what if the child will accompany by their grand parent and they are both applying for passport. Will the dfa gives a consideration about this?

    • Hi! I am not so sure about that, unless the grandparents are the legal guardians. Good luck with getting a passport for your child!

  11. Hi! How much po ba ang fee for affidavit of support and consent? Need po yan to get clearance sa dswd, right?

    • Hi, Emma! I’m afraid I do not know what the requirements are for DSWD. But the fee for the affidavit’s notarization depends on the lawyer or notary.

  12. Hi I just want to ask, my minor daughter was scheduled for passport application. Her father and I were not yet married. Do I need to bring a notarized affidavit of support and consent to travel? Since my daughter is traveling with me. Thanks.

    • Hi, Grace! We travelled with our children, but still brought the affidavit in case they ask for it.

    • Hi, M.Q.! I don’t know if the requirements are the same if you go to a consul or embassy outside of the country. You may want to check with the local office.

  14. Hi if i have an illegitimate child with a foreigner (but unfortunately i was married ten years ago but not yet annulled) and want to get my son a Philippine passport will i be allowed to do so? On my sons birthcert its my maiden name but my passport has my married name. Will this be an issue?

  15. Hi im a minor 17, im an illegitimate child. Sa nso ko mother ko lng ang nakalagay but im living with my father and his family now. Me and my sister — father side nagpaplano kami sumama kay tita (sister ng step mom ko) sa trip nila sa korea on april . nakapag set na kami ng date sa december 2 para sa application ng passport . ask ko lang po sa affidavit ano po ang pwedeng ilagay as a relation ng mother ko dun sa kapatid ng step mom ko? Sa dswd travel clearance din po ksi kaylangan sya. Sana po masagot nyo yung questions ko.
